NW Regional Council Care Coordinator
2019 TEN-STEP HOURLY RANGE: $23.50 – $30.66, DOQ
JOB SUMMARY: The Northwest Regional Council funds and/or provides community-based programs to help elders, people with disabilities, and people living with behavioral health disorders live in their own homes and communities for as long as possible, postponing or eliminating the need for institutional care.
A newly funded Care Coordinator position will be integrated in the Skagit Valley Hospital Care Management team in order to accelerate the development of cross-organizational care transitions and care coordination. The position will be an NWRC employee but operate out of the hospital offices, and will maintain operational linkages with the emergency department, hospital inpatient services, primary care, behavioral health, Health Homes, long-term care, community services and existing care management resources.
The service includes care coordination, care transitions, and linkage to community resources for patients within the hospital’s emergency department and inpatient setting for older adults and individuals with disabilities, with an emphasis on those who are eligible for Medicaid or are Dual Eligible (Medicare/Medicaid). In addition, the service includes following patients to home and other settings for transition care and to ensure longer-term stabilization.
Program outcomes include:
• Reduced unnecessary readmissions to the hospital
• Reduced utilization of the ED for low acuity conditions;
• Effective transitions to stable, supported living situations;
• Increased effective utilization of medical homes, behavioral health and long-term care; • Increased connectivity with existing care management systems in the community.
